Steps to Apply

1. Complete and submit your Application Form and Essay by the deadline.  Applications without a complete essay will not be considered for scholarship funding. 300 word minimum.  Carefully read the essay requirements before submitting. We will send all additional forms to you via email after we receive and accept your application form and essay.
   1. If you have trouble attaching the essay to the form, please complete and submit the rest of the electronic form, and then email the essay to cenursing@csuohio.edu.
2. Submit your Official High School Transcript, High School Diploma, or GED Certificate (email or fax).
3. Submit a copy of your Driver License/State ID and current medical/health insurance information (email or fax).
4. Sign and submit the Authorization to Release Information Form (email or fax).
5. Submit your COMPLETE Immunization Documentation (Photos or screenshots of documents are not accepted, PDF or paper only).
   1. Tuberculosis (TB) Test Results – Current negative test results or positive titer showing the antibodies in your blood.
   2. Hepatitis B – positive titer results or record of 3 booster series
   3. Varicella (Chicken Pox) – positive titer results or record of immunization
   4. Tetanus-Diphtheria (TDap) – Booster within past 10 years or positive titer results
   5. MMR (Measles, Mumps, Rubella) – positive titer or record of immunization
   6. Influenza Vaccination
   7. COVID Vaccination
6. Schedule your background check after you are officially accepted into the program.  Further instructions will be provided on acceptance.

Essay requirements:

Please upload a 300 word (minimum) essay describing all past/current service in your community, your future goals/plans and how this program fits into that plan, and how you will fit the 14 Saturday classes, 4 days of CDSM training, and 130 service hours into your current schedule.  If you have difficulty uploading your document with your application form, you may alternatively email to cenursing@csuohio.edu
