College of Health Student Petition Form

Overview  

The CSU Undergraduate College Petition Form is the required form for undergraduate students to submit petition requests to their assigned college/program.  This form cannot be used for University Petitions.  Students must use the CSU Undergraduate College Petition Form for the following requests:  

Undergraduate College Petitions Requests 

• Retroactive course(s) withdrawal after the semester deadline
• Late course(s) add after semester deadline
• Readmission to the University after academic suspension
• Credit overload (requesting to register for more than 19 credit hours in a term)  
• Extension of an Incomplete (requesting to extend a grade of Incomplete from a past term)  
• Reinstatement (requesting to restore courses previously dropped for non-payment)  
• Upper-Division Credit Waiver (requesting to graduate with fewer than the required minimum of 24 upper-division credit hours)
• GPA requirement waiver
• Foreign language requirement substitution or exemption  

Student Requirements 

• Review the CSU Petitions Guidelines to determine if your request is a college or university petition.
• Consult with your Academic Coach prior to submitting the Petition.
• Complete all required questions in the petition.
• Attach a narrative providing a clear rationale for your petition  
• Attached supporting documentation, as needed. 

Notification of Petition Committee Decisions 

Notification of petition decisions will be sent to your CSU student email account.  

• September - Petition Due Date - 9/4/26
• October - Petition Due Date - 10/2/26
• November - Petition Due Date - 11/6/26
• December - Petition Due Date - 12/4/26
• January - Petition Due Date - 1/15/27
• February - Petition Due Date - 2/5/27
• March - Petition Due Date - 3/5/27
• April - Petition Due Date - 4/2/27
• May - Petition Due Date - 5/7/27 
• June - Petition will not be accepted.
• July - Petition will not be accepted.
• August - Petition Due Date - 8/6/27

CARE Support 

Petitions for students with an acute medical crisis for which the student is not registered with ODTS or prepared to manage without support will be referred to the CARE team.
