Currently, Cleveland Clinic, University Hospital, ***MetroHealth Medical* Center, St. Vincent Charity Hospital and The Louis Stokes VA Medical Center, REQUIRES ALL** students and faculty to receive the COVID-19 vaccine in order to have clinical experiences at their facility.

*CSU students and faculty can obtain a COVID vaccine free of charge at the CSU Health and Wellness Services Center on Tuesdays 9:00 am-4:00 pm (Walk-ins, No appointments needed)

Please print out and complete the entire Program and Health Requirements packet. All documents need to be uploaded into Exxat for students admitted Spring 2023 to current. Students admitted prior to January 2023 should submit documents to the School of Nursing.
